At Kaur Case Management, our aim is to provide a gold-standard case management service to those
who have sustained life-changing injuries. We are an independent and bespoke case management
service founded upon 15 years of experience in Occupational Therapy and Case Management. Our
expertise includes brain injury, spinal cord injury, stroke, complex orthopedic injuries, and
psychological trauma.
We cover London and the surrounding areas.

Case Management

Case management is a collaborative process, that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet an individuals health and wellbeing, education and occupational needs using communication and available resources to promote quality, cost effective and safe outcomes. The service is typically delivered within a medico legal framework.

At Kaur Case Management, the case manager will complete an immediate needs assessment to identify the clients needs and make recommendations to meet these needs. The case manager will work with the client to agree goals and implement a plan which may involve commissioning a multidisciplinary to deliver treatment, developing a bespoke package of support or increasing a client’s participation in the community. The case manager will regularly review the rehabilitation programme, estimate costs and comprehensively report on progress to the client, family, professionals involved and legal team.
